Effects of sulfotanshinone sodium injection combined with ulinastatin on inflammation and vascular endothelial function of patients with severe acute pancreatitis

Abstract:Objective To observe the effects of sulfotanshinone sodium injection combined with ulinastatin on peripheral blood mononuclear cell NF-κB inflammatory signal pathway and vascular endothelial function in patients with severe acute pancreatitis.Methods Ninety-six patients with severe acute pancreatitis who were admitted and treated in our hospital from January 2014 to January 2016 were divided into observation group ( n =48) and control group ( n =48).The patients in control group ,on the basis of conventional therapy,were treated by ulinastatin,however,the ptients in observation group, on the basis of control group, were treated by sulfotanshinone sodium injection,with a treatment course of 2 weeks.The serum levels of interleukin 6 (IL-6), interleukin 8 (IL-8), tumor necrosis factorα(TNF-α), ultra-sensitivity C-reactive protein (hs-CRP), nitric oxide (NO), von Willebrand Factor (vWF), thromboxane A2 (TXA2) and prostacyclin (PGI2) were detected by ELISA.The expression levels of key proteins ( NF-κB and IκB) of peripheral blood mononuclear cell NF-κB inflammatory signal pathway were detected by Real time-PCR and Western-Blot,respectively.The changes of hemorheological parameters including plasmaviscosity, high blood viscosity, low blood viscosity, hematocrit and RBC aggregation index were measured by semi-automatic blood rheometer.The patients'prognoses were evaluated by means of APACHEⅡ and Balthazar CT scoring system.Results As compared with those before treatment, the levels of IL-6,IL-8,TNF-α,hs-CRP,NF-κB mRNA and protein,vWF,TXA2, plasmaviscosity, high blood viscosity, low blood viscosity, hematocrit and RBC aggregation index, APACHEⅡscores,Balthazar CT scores were significantly decreased after treatment in both groups,however, the expression levels of IκB mRNA and protein,NO,PGI2 were significantly decreased after treatment in both groups ( P <0.05).However the improvement degree of these indexes in observation group was more obvious than that in control group ( P <0.05 ) . Conclusion The sulfotanshinone sodium injection combined with ulinastatin can inhibit the activation of peripheral blood mononuclear cell NF-κB inflammatory signal pathway in patients with severe acute pancreatitis and can reduce the release of inflammatory factors, and can improve vascular endothelial function and hemorheological parameters, which may be one of action mechanisms of sulfotanshinone sodium injection.
