The diagnostic value of combination detection of serum levels of VEGF, CysC and Hcy in patients with diabetic nephropathy

Abstract：Objective To observe the expressions of serum vascular endothelial growth factor ( VEGF ) , cystatin (CysC), homocysteine (Hcy) in patients with diabetic nephropathy (DN), and to investigate the intervention effects of piperazine ferulate combined with irbesartan on DN .Methods A total of 87 patients with type 2 diabetes mellitus who were admitted and treated in our hospital from January 2013 to January 2015 were divided into normal albuminuria group ( n =32), microalbuminuria group ( n =25) and macroalbuminuria group ( n =30) according to clinical staging,moreover, the other 35 healthy subjects were served as control group .The patients in microalbuminuria group and macroalbuminuria group were treated by piperazine ferulate combined with irbesartan .The serum levels of VEGF were detected by ELISA .The levels of fasting blood glucose, glycosylated hemoglobin A1c (HbA1c),creatinine, urea,CysC and Hcy were detected by automatic biochemistry analyzer .Results As compared with those in control group , the serum levels of VEGF ,CysC,Hcy were gradually increased in normal buminuria group , microalbuminuria group and macroalbuminuria group ( P <0.05).The serum levels of VEGF were positively correlated to those of CysC ,Hcy in patients with diabetes mellitus ( P <0.05),moreover,which were positively correlated to those of fasting blood glucose , glycosylated hemoglobin A1c (HbA1c),creatinine,urea ( P <0.05). The piperazine ferulate combined with irbesartan could significantly reduce proteinuria and could decrease the serum levels of VEGF, CysC and Hcy ( P <0.05).Conclusion The high-expression of VEGF,CysC,Hcy exists in patients with DN,and combination detection of VEGF ,CysC and Hcy can enhance the diagnostic sensitivity for DN .Moreover the piperazine ferulate combined with irbesartan has protective effects on renal function by inhibiting neovascularization .
