Comparison study on intensity-modulated conformal radiotherapy and three-dimensional conformal radiotherapy in treatment of cervical cancer

Abstract:Objective To investigate and compare the protective effects of intensity-modulated conformal radiotherapy (IMRT) and three-dimensional conformal radiotherapy (3DCRT) on patient’ s trachea in treatment of cervical cancer. Methods Fourteen patients with cervical cancer (Ⅱb ~Ⅲb ) who were treated in our hospital from February 2012 to February 2013 were enrolled in the study .All the patients underwent routine external and intracavitary radiotherapy , before radiotherapy,they received CT scan, sketching target area,and CTV (clinical target area) included cervical, uterus, vagina and other areas as well as surrounding tissues , PTV ( planning target area ) took CTV as basis to prolong 5mm outwards to form PTV.The differences of volume and dosage of trachea receiving radiation were analyzed and compared between two groups by means of HVH image analysis .Results DVH image analysis showed that the exposure volume at 45Gy,54Gy, 63Gy in bladder , small intestine and rectum by IMRT was significantly lower than that by DCRT , moreover , with the increase of prescription dosage , the exposure volume and dosage in bladder , small intestine and rectum were increased accordingly . Conclusion The protective effecets on risk organs during treatment by IMRT in patients with cervical cancer (Ⅱb~Ⅲb) are superior to those by 3DCRT,and the advantage of IMRT is that the exposure volume of risk organs is decreased with the enlargement of CTV,with exact location.
