Effects of rosuvastatin on Toll-like receptor 4 expression in peripheral blood monouclear cell and on inflamma tory factor in patients with acute myocardial infarction after selective percutaneous coronary intervention

Abstract:Objective To observe the change of Toll-like receptor 4 ( TLR4) in acute myocardial infarction ( AMI) patients undergoing selective percutaneous coronary intervention ( PCI ) and to investigate the effects of rosuvastatin on Toll-like receptor 4 expression in peripheral blood monouclear cell and on inflammatory factor.Methods A total of 90 patients with AMI treated by PCI were enrolled in the study .Meanwhile 70healthy subjects were served as control group .Real time-PCR and Western Blot were used to detect the expression levels of TLR 4 mRNA and protein.Gensini score was used to evaluate the severity of coronary artery lesions .The 90patients were randomly divided into two groups :group A and group B, with 45 patients in each group.The patients in both groups received routine therapy ,however,the patients in group B,on the basis of routine therapy, were given orally rosuvastatin (10 mg/d) on 7 days before PCI.The changes of LVEF, LVESD, LVEDD, TLR4 mRNA and protein, hs-CRP, TNF-α, IL-6 were detected before surgery and on 7d after surgery.Results As compared with those in control group,the expression levels of TLR4 mRNA and protein were significantly increased in patients with AMI ( P <0.05).Moreover the expression levels of TLR4 mRNA and protein were gradually increased,with the increasing of severity of coronary lesions ( P <0.05).The correlation analysis showed that the expression levels of TLR4 mRNA and protein were positively correlated to the severity of coronary lesions ( P <0.05).As compared with that before therapy,LVEF in both treatemt groups were significantly increased on 7d after PCI ( P <0.05) ,furthermore, LVEF in group B was significantly higher than that in group A ( P <0.05 ) .The levels of LVESD, LVEDD, TLR4 mRNA and protein, hs-CRP, TNF-α, IL-6 in both treatment groups were significantly decereased on 7d after PCI,as compared with those before surgery ( P <0.05),moreover, which in group B were significantly lower than those in group A ( P <0.05).C onclusion The short-term application of rosuvastatin before elective PCI surgery in patients with AMI can obviously improve patient’ s cardiac function,and its action mechanism may be related with inhibiting the activation of TLR4 signal transduction pathway.
