The predictive value of neutrophil/lymphocyte ratio and hypersensitive C-reactive protein for contrast-induced nephropathy after percutaneous coronary intervention in patients with NSTEMI
Abstract:Objective To investigate the predictive value of neutrophil /lymphocyte ratio ( NLR ) and hypersensitive C-reactive protein(hsCRP)for contrast-induced nephropathy (CIN)after percutaneous coronary intervention (PCI)in patients with non-ST-segment elevation myocardial infarction (NSTEMI).Methods The 400 patients undergoing PCI were enrolled in the study.CIN was defined as an increase in serum creatinine more than 44.2μmol/L or 25%from baseline within 48~72h after contrast exposure .Baseline data were compared among between CIN group and non CIN group by means of T-test, and the correlation between NLR/hsCRP and CIN was evaluated by multivariate Logistic regression analysis .Results The levels of NLR and hsCRP were significantly increased in CIN group .Renal insufficiency and diabetes mellitus were closely correlated to CIN,and advanced age , diabetes, reduced glomerular filtration rate , large dose of contrast media and heart failure were independent risk factors for CIN.NLR>5.1 and hsCRP>16mg/dl were also independent risk factors of CIN .Conclusion NLR and hsCRP may be simple and reliable predictive indexes for CIN in patients with NSTEMI after percutaneous coronary intervention .
