The applied research on Thinprep cytology test in detecting ASCUS cases

Abstract：Objective To explore the interrelationship of detection results among Thinprep cytology test ( TCT ) , HC-Ⅱtest,histopathologic diagnosis ,high-risk HPV test in examining atypical squamous cells of undetermined significance (ASCUS) cases in order to provide reference for enhancing the levels of early screening of cervical cancer .Methods A total of 82 385 women who received cervical screening voluntarily in Health Care Institute for Maternity and children of Qinhuangdao City from November 2010 to November 2013 were enrolled in the study in which 1668 patients were diagnosed as ASCUS by TCT,who were examined by HC-Ⅱ test, HPV-DNA detection and histopathologic diagnosis .Results Among 82 385 specimens,negative for intraepithelial lesion or malignancy (NILM) accounted for 97.25% (80 120/82 385); high grade squamous intraepithelial lesion (HSIL) accounted for 0.26% (213/82 385);low grade squamous intraepithelial lesion (LSIL) accounted for 0.34%(276/82 385);atypical squamsus cells that could not exclude HSIL (ASC-H) accounted for 0.08%(73/82 385);ASCUS accounted for 2.02% (1 668/82 385); atypical glandular cells of undetermined significance (AGUS) accounted for 0.05%(35/82 385).The detection positive rate of HC-Ⅱin ASCUS patients was 68.41%(1 141/1 668 );as compared with the ASCUS cases diagnosed by cytology ,those cases diagnosed by histopathology were inflammation changes,mollusciform wart/CINI degree,CINⅡ/Ⅲ degree,invasive squamous cell carcinoma ,and their proportion was in order 45.86%(765/1 668),42.21% (704/1 668),11.87% (198/1 668),0.06% (1/1 668).Conclusion High risk HPV detection should combined with LCT technique in screening of early cervical cancer and precancerous lesion ,moreover, the morphology of ASCUS should be studied further .
