Analysis of factors influencing the success of ultrasound-guided external cephalic version in term pregnant women during the third trimester

Abstract:Objective To analyze factors influencing the success of ultrasound-guided external cephalic version(ECV)per-formed in term pregnant women during the third trimester,in order to provide a reference for clinical treatment.Methods Seventy-one pregnant women who underwent ECV and were followed until delivery at Bengbu First People's Hospital from March 2021 to December 2024 were included.According to whether ECV was successful,patients were allocated to a suc-cess group(n=47)and a failure group(n=24).Clinical data of all mothers and neonates were collected.Multivariable lo-gistic regression analysis was used to analyze factors affecting ECV outcome.Results Among the 71 women who underwent ECV,47 were successful and 24 failed,with a success rate of 66.20%.Overall,there were 43 vaginal deliveries and 28 ce-sarean deliveries,yielding a final vaginal delivery rate of 60.56%.In the success group,43 women(91.49%)delivered va-ginally and 4(8.51%)by cesarean;all women in the failure group underwent cesarean delivery.The complication rate in the success group was significantly lower than that in the failure group(8.51%vs.33.33%;P<0.05).Compared with the failure group,the success group had higher proportions of multiparas,non-anterior placentation,higher amniotic fluid index(AFI),use of neuraxial anesthesia,and pre-procedure use of tocolytics;differences were all statistically significant(P<0.05).Multivari-able logistic regression analysis showed that non-anterior placentation(OR=0.119,95%CI:0.025-0.562),use of neuraxial an-esthesia(OR=0.186,95%CI:0.040-0.861),and pre-procedure use of tocolytics(OR=0.148,95%CI:0.033-0.659)were factors influencing ECV outcome(P<0.05).Conclusion For pregnant women without contraindications to vaginal de-livery,ultrasound-guided ECV can help improve the delivery experience and increase the vaginal delivery rate,and provides theoretical evidence for assessing the safety and effectiveness of ECV for breech or transverse presentations in strengthening pregnancy outcomes.
