Relationship between C-reactive protein-triglyceride glucose index and lower limb atherosclerosis in perimenopausal women with type 2 diabetes

Abstract:Objective To explore the relationship between C-reactive protein-triglyceride glucose index(CTI)and lower limb atherosclerosis in perimenopausal women with type 2 diabetes(T2DM),aiming to provide a reference for clinical decision-making.Methods A total of 218 perimenopausal women with T2DM diagnosed between August 2021 and December 2024 at our hospital were selected as the study group.Additionally,100 perimenopausal healthy women who underwent physical examinations at the hospital during the same period were selected as the control group.Levels of C-reactive protein(CRP),fasting plasma glucose(FPG),triglycerides(TG),and other indices were measured,and triglyceride glucose(TyG)index and CTI were calculated.The study group underwent lower limb color Doppler ultrasound,and based on the presence or ab-sence of lower limb atherosclerosis,they were assigned to the atherosclerosis group(n=96)and non-atherosclerosis group(n=122).The predictive value of CTI for lower limb atherosclerosis in perimenopausal T2DM women was analyzed using the receiver operating characteristic(ROC)curve,and multivariate logistic stepwise regression analysis was conducted to i-dentify the influencing factors for lower limb atherosclerosis in these women.Results CRP,TyG index,and CTI in the study group were significantly higher than those in the control group(P<0.05).In the atherosclerosis group,CRP,TyG index,and CTI were significantly higher than those in the non-atherosclerosis group(P<0.05).ROC curve analysis showed that the area under the curve(AUC)for CTI in predicting lower limb atherosclerosis in perimenopausal T2DM women was 0.905,significantly higher than the AUCs of CRP(0.746)and TyG index(0.872)alone(Z=10.729,9.481,P<0.001).Univariate analysis showed no statistically significant differences between the atherosclerosis and non-athero-sclerosis groups in age,body mass index(BMI),diastolic blood pressure,systolic blood pressure,neutrophil count(NE),alanine aminotransferase(ALT),aspartate aminotransferase(AST),total cholesterol(TC),high-density lipoprotein cho-lesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),and the proportion of patients with a smoking or drinking history(P>0.05).The atherosclerosis group had a longer duration of T2DM,higher waist-to-hip ratio,higher glycated hemoglobin(HbA1c),higher white blood cell count(WBC),and higher TG compared to the non-atherosclerosis group,with all differences being statistically significant(P<0.05).Multivariate logistic stepwise regression analysis showed that high waist-to-hip ratio(OR=2.617,95%CI:1.491-4.593),high HbA1c(OR=2.291,95%CI:1.374-3.821),and CTI ≥ 9.77(OR=2.983,95%CI:1.740-5.114)were independent risk factors for lower limb atherosclerosis in perim-enopausal women with T2DM(P<0.05).Conclusion Elevated CTI is associated with the occurrence of lower limb athero-sclerosis in perimenopausal women with T2DM and is an independent influencing factor for lower limb atherosclerosis in these women.Moreover,CTI has high predictive value for the occurrence of lower limb atherosclerosis in perimenopausal women with T2DM.
