Effect of the psychological-behavioral trajectory recording method combined with cognitive-behavioral stress management on patients with rheumatic heart disease after valve replacement

Abstract:Objective To investigate the effect of the psychological-behavioral trajectory recording method combined with cognitive-behavioral stress management on the psychological status and quality of life of patients with rheumatic heart disease(RHD)after valve replacement,and to provide a clinical reference.Methods From July 2023 to June 2024,130 RHD pa-tients who underwent valve replacement at the First Affiliated Hospital of Air Force Medical University were enrolled.Pa-tients were assigned to a routine care group and a combined care group using a random number table,with 65 cases in each.The routine care group received conventional management;the combined care group received the psychological-behavioral trajectory recording method combined with cognitive-behavioral stress management.Disease cognition(Champion Health Belief Model Scale[CHBMS]),psychological status(Depression Anxiety Stress Scale[DASS]),quality of life(Generic Quality of Life Inventory[GQOLI]),and the incidence of post-discharge complications were compared between the two groups.Results There were no statistically significant differences between the two groups before intervention in CHBMS of susceptibility,seriousness,benefits,barriers,health motivation,and self-efficacy(P>0.05).At 1 and 3 months after in-tervention,dimensional scores for susceptibility,seriousness,benefits,barriers,health motivation,and self-efficacy on the CHBMS were higher in both groups than before intervention,and the combined care group scored higher than the routine care group;these differences were statistically significant(P<0.05).Before intervention,there were no statistically signifi-cant differences between the two groups in DASS scores for depression,anxiety,and stress(P>0.05).At 1 and 3 months after intervention,DASS scores for depression,anxiety,and stress in both groups were lower than before intervention,and the combined care group's scores were lower than those of the routine care group;the differences were statistically signifi-cant(P<0.05).Before intervention,there were no statistically significant differences between the two groups in GQOLI scores for physical function,psychological function,social function,and material living conditions(P>0.05).At 1 and 3 months after intervention,GQOLI scores for physical function,psychological function,social function,and material living conditions in both groups were higher than before intervention,and the combined care group's scores were higher than those of the routine care group;the differences were statistically significant(P<0.05).The incidence of post-discharge complica-tions in the combined care group was significantly lower than that in the routine care group(P<0.05).Conclusion The psychological-behavioral trajectory recording method combined with cognitive-behavioral stress management can promote disease cognition,improve psychological status,raise quality of life,and help prevent complications in RHD patients after valve replacement,warranting clinical promotion and application.
