Factors influencing postpartum pelvic floor dysfunction in primiparous women after vaginal delivery

Abstract:Objective To explore factors influencing the occurrence of pelvic floor dysfunction(PFD)in primiparous women af-ter vaginal delivery,providing a reference for clinical practice.Methods From January to August 2024,908 primiparas who delivered successfully at the Affiliated Mianyang Hospital of University of Electronic Science and Technology of China and un-derwent pelvic floor function screening at the same hospital were enrolled.According to pelvic floor electrophysiological test re-sults,participants were assigned to a normal group and an abnormal group.Pelvic floor electrophysiology was assessed by the same pelvic floor therapist at 6 months postpartum.Multinomial logistic regression was used to analyze factors associated with PFD.Results Among the 908 vaginally delivered primiparas,pelvic floor electrophysiology showed pelvic dynamic pressure abnormalities in 786 cases(86.56%);type Ⅰ muscle fiber strength abnormalities in 780 cases(85.90%);type Ⅰ muscle fiber fatigue in 789 cases(86.89%);type Ⅱ muscle fiber strength abnormalities in 648 cases(71.37%);and type Ⅱ muscle fiber fa-tigue in 856 cases(94.27%).Multinomial logistic regression analysis indicated that second-stage labor duration was an inde-pendent risk factor for pelvic dynamic pressure abnormality;perineal tear was a risk factor for type Ⅰ muscle fiber strength ab-normality;increased gestational weight gain was an independent risk factor for type Ⅰ muscle fiber fatigue;perineal tear was a risk factor for type Ⅱ muscle fiber strength abnormality;and both second-stage labor duration and perineal tear were independ-ent risk factors for type Ⅱ muscle fiber fatigue(P<0.05).Conclusion Gestational weight gain,perineal tear,and second-stage labor duration are associated with PFD in women after vaginal delivery.It is recommended to provide pregnant women with guidance on nutrition and exercise during pregnancy,to avoid perineal tears where possible,and to initiate early postpar-tum pelvic floor muscle rehabilitation to reduce the incidence of PFD.
