Predictive efficacy of serum hs-CRP,Hcy,and NLR for in-stent restenosis after PCI in coronary heart disease patients

Abstract:Objective To analyze the predictive efficacy of serum high-sensitivity C-reactive protein(hs-CRP),homocysteine(Hey),and neutrophil-to-lymphocyte ratio(NLR)for in-stent restenosis(ISR)after percutaneous coronary intervention(PCI)in coronary heart disease patients.Methods A total of 105 coronary heart disease patients who underwent PCI with at least one drug-eluting stent(DES)at a hospital in Shandong province from February 2022 to October 2024 were enrolled.Based on the presence or absence of ISR occurrence within 6 months post-PCI,patients were assigned to ISR(n=13)and non-ISR(n=92)groups.General data and admission serum hs-CRP,Hcy,and NLR were compared.Logistic regression analysis was used to analyze independent correlations of hs-CRP,Hcy,and NLR,and receiver operating characteristic(ROC)curves were em-ployed to evaluate the predictive efficacy of combined detection of hs-CRP,Hcy,and NLR.Results No significant differences were found in gender,age,body mass index(BMI),left ventricular ejection fraction(LVEF)score on admission,or proportions of hypertension,dyslipidemia,smoking history,or multi-vessel disease between groups(P>0.05).Admission serum hs-CRP([3.51±1.22]mg/L vs.[2.03±0.98]mg/L),Hcy([16.88±3.85]μmol/L vs.[12.57±3.01]μmol/L),and NLR(3.62±1.15 vs.2.38±0.91)were significantly higher in the ISR group than in the non-ISR group(P<0.05).Multivariate analysis showed hs-CRP(OR=1.550,95%CI:1.087-2.209),Hey(OR=1.190,95%CI:1.031-1.373),and NLR(OR=1.820,95%CI:1.098-3.018)on admission were independent risk factors of ISR(P<0.05).ROC curves demonstrated that combined hs-CRP,Hcy,and NLR prediction achieved 92.30%sensitivity and 87.00%specificity,with an area under the curve(AUC)of 0.947(95%CI:0.899-0.995).Conclusion Elevated serum hs-CRP,Hcy,and NLR on admission are independent risk factors for ISR within 6 months post-PCI in coronary heart disease patients.Combined detection offers high predictive value for ISR,providing refer-ences for preoperative risk assessment.
