Risk factors for unplanned extubation in tracheally intubated patients in a neurology ICU

Abstract:Objective To investigate risk factors for unplanned extubation(UEX)in tracheally intubated patients in a neurol-ogy intensive care unit(ICU),in order to provide evidence for clinical prevention of UEX.Methods We retrospectively in-cluded 382 tracheally intubated patients admitted to the Neurology ICU of Tangdu Hospital of Air Force Medical University from January 2022 to October 2024.Patients were assigned to a UEX group and a non-UEX group according to whether UEX occurred.Data collected included a general information form,Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ)scores,assessment of consciousness status scores,and the Richmond Agitation-Sedation Scale(RASS)scores;the use of physical restraints and sedatives during ICU stay was also recorded.Results Forty-one patients experi-enced UEX,with an incidence of 10.73%.Multivariate logistic regression analysis showed that delirium(OR=4.166),RASS score+2 to+4(OR=5.099),use of restraints(OR=0.213),and use of sedatives(OR=0.075)were influencing factors associated with UEX in tracheally intubated patients in the neurology ICU(P<0.05).Conclusion For tracheally intubated patients in a neurology ICU,clinicians should pay particular attention to those with delirium and RASS scores of+2 to+4.When appropriate,standardized implementation of physical restraints or administration of sedatives may be necessary to reduce the incidence of UEX.
