Development and psychometric evaluation of the nursing needs scale for patients with emergency acute ischemic stroke

Abstract:Objective To develop a nursing needs scale for patients with emergency acute ischemic stroke(AIS)and conduct a psychometric evaluation.Methods The initial scale was developed through literature review,semi-structured interviews,and expert consultations.A pre-test of the scale was conducted with 352 AIS patients admitted to the Third People's Hospi-tal of Chengdu from March to October 2023.A formal psychometric test was performed with 526 AIS patients admitted to the hospital between November 2023 and November 2024.Re-test reliability was evaluated with 50 AIS patients admitted between November 2023 and November 2024.Results The nursing needs scale for emergency AIS patients consisted of 4 dimensions and 22 items.Exploratory factor analysis showed that the KMO value was 0.848,and Bartlett's test of spherici-ty resulted in a x2 value of 10,151.771(P<0.001).Confirmatory factor analysis indicated a x2/df value of 1.345,root mean square residual(RMR)of 0.071,incremental fit index(IFI)of 0.778,Tucker-Lewis index(TLI)of 0.884,compar-ative fit index(CFI)of 0.887,goodness of fit index(GFI)of 0.915,and root mean square error of approximation(RM-SEA)<0.08.The scale's convergent and discriminant validity showed that the standardized factor loadings of the 4 dimen-sions ranged from 0.785 to 0.952;the composite reliability(CR)values for the 4 dimensions were 0.922 5,0.931 8,0.909 4,and 0.887 1,respectively;the average variance extracted(AVE)values were 0.665 3,0.733 2,0.668 3,and 0.571 3,respectively.The square roots of the AVE values were 0.815,0.856,0.818,and 0.756,respectively.The corre-lations between the 4 dimensions were statistically significant(P<0.001),and the square roots of the AVE for each dimen-sion were higher than the inter-dimensional correlations.Reliability analysis revealed that the overall Cronbach's α coefficient for the scale was 0.894;the Cronbach's α for the re-test was 0.903;the Cronbach's α for each of the 4 dimensions was grea-ter than 0.7.Split-half reliability showed that the split-half correlation coefficient for the scale was 0.909,with significant correlation(P<0.01).Conclusion The nursing needs scale for emergency AIS patients demonstrates good reliability and validity.This scale provides a basis for assessing,diagnosis,treatment,and nursing of emergency AIS patients,offering a scientific evaluation basis for the implementation of targeted nursing interventions.
