Prognostic value of systemic immune-inflammation index in acute coronary syndrome patients after percutaneous coronary intervention

Abstract:Objective To investigate the prognostic value of systemic immune-inflammation index(SII)in acute coronary syn-drome(ACS)patients after percutaneous coronary intervention(PCI).Methods A total of 150 ACS patients undergoing PCI at Xichang People's Hospital from January 2018 to January 2023 were enrolled.Clinical data included age,sex,body mass index,ACS type,number of stents implanted,smoking history,hypertension history,diabetes history,duration of postoperative anticoagulant use,left ventricular ejection fraction(LVEF)score before PCI,low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG),total cholesterol(TC),and neutrophil count,platelet count,and lymphocyte count after PCI.SII was calculated from platelet,neutrophil,and lymphocyte counts.Based on the presence or absence of major adverse cardiovascular events(MACE)during follow-up,patients were allocated to the good prognosis(absence of MACE)and poor prognosis(presence of MACE)groups.Receiver operating characteristic(ROC)curves were employed to analyze SII's prog-nostic value of ACS patients undergoing PCI;multivariate stepwise logistic regression was adopted to identify influencing fac-tors of patients'prognoses.Results The good prognosis group had 108 patients and the poor prognosis group had 42 patients.SII was significantly lower in the poor prognosis group than in the good prognosis group(P<0.05).The area under the curve(AUC)of SII for predicting prognosis was 0.946(95%CI:0.901-0.996),with a cutoff of 594.59,a sensitivity of 87.24%,and a specificity of 88.39%.The poor prognosis group had higher proportions of patients with≥3 implanted stents and smoking history,higher neutrophil and platelet levels,but lower lymphocyte counts than the good prognosis group,with statistically significant differences(P<0.05).Multivariate analysis identified number of stents(OR=2.762;95%CI:1.510-5.051),neutrophil count(OR=2.063;95%CI:1.089-3.908),platelet count(OR=1.806;95%CI:1.148-2.840),lympho-cyte count(OR=2.575;95%CI:1.473-4.502),and SII(OR=4.836;95%CI:2.665-8.774)as influencing factors of post-PCI prognosis for ACS patients(P<0.05).Conclusion SII is elevated in ACS patients with poor post-PCI prognosis.SII dem-onstrates better efficacy in evaluating post-PCI prognosis in patients with ACS.Furthermore,stent number,neutrophil,plate-let,lymphocyte,and SII levels are influencing factors of post-PCI prognosis.
