Predictive value of(neutrophils+monocytes)/lymphocytes ratio for major adverse cardiovascular events in coronary heart disease patients combined with heart failure

Abstract:Objective To investigate the predictive value of(neutrophils+monocytes)/lymphocytes ratio(NMLR)for major adverse cardiovascular events(MACE)in coronary heart disease(CHD)patients combined with heart failure,providing refer-ences for clinical decision-making.Methods A total of 104 CHD patients combined with heart failure admitted to a hospital in Shandong Province from December 2022 to December 2023 were enrolled.Baseline/clinical data and admission NMLR were collected.Based on the 1-year follow-up MACE occurrence,patients were allocated to the MACE and non-MACE groups.Re-ceiver operating characteristic(ROC)curves were plotted to analyze NMLR's predictive value for MACE;multivariate logistic regression was adopted to identify MACE risk factors.Results NMLR was significantly higher in the MACE group(6.57±1.39)than in the non-MACE group(3.12±0.78;t=16.238,P<0.001).The area under the curve(AUC)(95%CI)of NMLR for predicting MACE among CHD patients combined with heart failure was 0.916(0.871-0.966),with a sensitivity of 86.91%and a specificity of 85.13%at a cutoff of 5.15.Univariate analysis showed no significant differences in body mass index,gender distribution,heart rate,systolic/diastolic blood pressure,left ventricular ejection fraction,NYHA or Killip functional classification,percutaneous coronary intervention history,history of smoking,history of alcohol consumption,or comorbidities(hypertension/diabetes/hyperlipidemia)between groups(P>0.05).Age,creatinine,and uric acid levels were significantly higher in the MACE group than in the non-MACE group(P<0.05).Multivariate stepwise logistic regression i-dentified age(OR=2.252,95%CI:1.278-3.969)and NMLR(OR=3.684,95%CI:1.870-7.258)as influencing fac-tors of MACE among CHD patients combined with heart failure(P<0.05).Conclusion Elevated NMLR is a predictive factor of MACE in CHD patients combined with heart failure and may identify high-risk populations.
