Clinical observation of OT combined with PNF in treating shoulder-hand syndrome after stroke

Abstract:Objective To evaluate the clinical efficacy of occupational therapy(OT)combined with proprioceptive neuromus-cular facilitation(PNF)in treating shoulder-hand syndrome after stroke(SHSAS),providing evidence for clinical manage-ment.Methods One hundred five SHSAS patients admitted to Ma'anshan Hospital of Traditional Chinese Medicine(Janu-ary 2022-January 2024)were randomized into the OT group(n=51)and combination group(n=54).Both groups re-ceived basic treatments including blood pressure,blood glucose,and blood lipid management,cerebral circulation improve-ment,and neurotrophic therapy.The OT group received occupational therapy alone,while the combination group received additional PNF rehabilitation.Treatment lasted 8 weeks.Outcomes included Visual Analog Scale(VAS)scores,shoulder joint,hand joint peak torque(PT),shoulder joint range of motion(ROM),Modified Ashworth Scale(MAS)scores,and Fugl-Meyer Assessment(FMA)scores before and after treatment.Results Pre-treatment shoulder and hand VAS scores showed no significant differences(P>0.05).Post-treatment,both groups exhibited significantly reduced VAS scores(P<0.05),with the combination group demonstrating significantly lower scores than the OT group(P<0.05).Shoulder ROM[forward flexion(FE),extension(E),abduction(AB),internal rotation(IR),external rotation(ER)]showed no baseline differences(P>0.05).Post-treatment,all shoulder ROM angles(FE,E,AB,IR,and ER)significantly increased in both groups(P<0.025),with greater improvement in the combination group(P<0.025).Shoulder abductor/flexor and hand extensor/flexor PT values showed no significant differences compared with those pre-treatment(P>0.05),but indicated a significant increase post-treatment in both groups(P<0.025),with the combination group outperforming the OT group(P<0.025).Pre-treatment MAS and FMA scores showed no significant differences(P>0.05).Post-treatment MAS scores decreased significantly in both groups,with lower values in the combination group than the OT group(P<0.05),while FMA scores increased significantly,with higher values in the combination group than the OT group(P<0.05).Con-clusion OT combined with PNF markedly alleviates pain/swelling,ameliorates limited joint mobility,promotes joint ROM and muscle strength,reduces spasticity,and strengthens upper limb motor function in SHSAS patients.
