Abstract:Objective To evaluate the clinical efficacy of Xuebijing acupoint injection combined with Huangma Tincture exter-nal application for diabetic foot ulcers(DFU),providing evidence for clinical management. Methods One hundred DFU pa-tients from outpatient or inpatient departments at Yubei District Hospital of Traditional Chinese Medicine(January-Sep-tember 2024)were randomized into four groups(n=25 each):control group(routine wound dressing on local ulcers),Xue-bijing group(routine dressing combined with Xuebijing acupoint injection),Huangma Tincture group(routine dressing combined with Huangma Tincture-soaked gauze),and experimental group(routine dressing plus combined Xuebijing and Huangma Tincture interventions).Outcomes that were compared included:overall response rates at 2,6,and 12 weeks;ulcer healing rate at 12 weeks;transcutaneous oxygen pressure(TcpO2)and ankle-brachial index(ABI)at baseline and 2,6,and 12 weeks;C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and TCM syndrome scores at baseline and 2,6,and 12 weeks;and time to complete ulcer healing. Results The experimental group demonstra-ted significantly higher overall response rates at all time points compared to all other groups, while both the Xuebijing and Huangma groups significantly surpassed the control group(P<0.05).Ulcer healing rates were highest in the experimental group, with those in Xuebijing and Huangma Tincture groups significantly higher than the control group(P<0.05).Com-plete healing time was shortest in the experimental group, with Xuebijing and Huangma Tincture groups healing significant-ly faster than controls(P<0.05).Huangma Tincture group incurred significantly higher treatment costs than others(P<0.05).TcpO2 and ABI levels were significantly elevated in the experimental group versus other groups at all time points, and higher in Xuebijing/Huangma Tincture groups versus controls(P<0.05).CRP,IL-6,and TNF-α levels were lowest in the experimental group versus other groups at all time points, with Xuebijing and Huangma Tincture groups lower than controls(P<0.05).TCM syndrome scores were most reduced in the experimental group, those in Xuebijing and Huangma Tincture groups were lower than the control group(P<0.05).Conclusion Combined Xuebijing acupoint injection with Huangma Tincture application significantly improves DFU outcomes, accelerates healing, reduces costs, and warrants clini-cal adoption.
