Comparative analysis of anesthetic effects between nalbuphine and fentanyl combined with propofol in painless colonoscopic polypectomy

Abstract:Objective To comparatively analyze the anesthetic effects of nalbuphine or fentanyl combined with propofol in painless colonoscopic polypectomy,aiming to provide references for clinical decision-making.Methods A total of 196 pa-tients undergoing painless colonoscopic polypectomy at a hospital in Shaanxi Province from May 2021 to March 2024 were selected as study participants.Participants were assigned to a study group(n=98)and a control group(n=98)using a ran-dom number table.The control group received fentanyl combined with propofol anesthesia,while the study group received nalbuphine combined with propofol anesthesia.Anesthesia induction time,propofol dosage,postoperative awakening time,anesthetic efficacy,adverse reactions,mean arterial pressure(MAP),pulse oxygen saturation(SpO2)at different time points[before anesthesia(T0),at loss of consciousness(T1),at colonoscope insertion(T2)],and Visual Analogue Scale(VAS)pain scores at 10 minutes and 30 minutes after awakening were observed and compared between the two groups.Re-sults No statistically significant differences were found in anesthesia induction time,propofol dosage,or postoperative awak-ening time between the study and control groups(P>0.05).The overall anesthetic response rate in the study group was signifi-cantly higher than that in the control group(98.98%vs.91.84%;P<0.05).Significant differences in MAP and SpO2 were ob-served between groups(Fgroup=122.36,132.69;P<0.05)and across time points(Ftime=131.02,135.34;P<0.05),with sig-nificant interaction effects between time and group(Finteraction=36.65,38.74;P<0.05).Specifically,compared with T0,MAP and SpO2 significantly decreased at T1 and T2 in both groups(P<0.05);no significant intergroup differences were noted at T0(P>0.05),but the study group showed significantly higher MAP and SpO2 than the control group at T1 and T2(P<0.05).VAS scores at 10 minutes and 30 minutes after awakening were significantly lower in the study group than in the control group(P<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group(4.08%vs.14.28%;P<0.05).Conclusion Nalbuphine combined with propofol demonstrates superior anesthetic efficacy in painless colonoscopic polypectomy,enhancing analgesic effects,maintaining stable intraoperative vital signs,and effectively reducing complication rates,making it safe for clinical application.
