Disease cognition,treatment adherence status,and influencing factors among adult psoriasis patients in Gansu region

Abstract:Objective To investigate the current status of disease cognition and treatment adherence among adult psoriasis pa-tients in Gansu region,analyze key influencing factors,and propose improvement strategies.Methods A convenience sam-pling method was used to enroll 502 psoriasis patients at the outpatient and inpatient service of the Department of Dermatol-ogy of a grade A tertiary hospital in Gansu(December 2023-June 2024).Data were collected via questionnaires:General Information Questionnaire,Disease Cognition Questionnaire(DCQ),and Morisky Medication Adherence Scale-8(MMAS-8).Descriptive statistics,univariate analysis,and multiple linear regression were employed to examine baseline characteris-tics and influencing factors of disease cognition and treatment compliance.Results The mean DCQ score was 16.03±4.38,and the MMAS-8 score was 4.51±2.25.The results of regression analysis showed that age at first diagnosis,education lev-el,preference for treatment,type of residence,and personal monthly income were the influencing factors of the respond-ents'disease cognition questionnaire scores(P<0.05),of which living in urban areas was the main positive influencing fac-tor and preference for oral Western medicine was the main negative influencing factor;disease in the stationary phase was the positive influencing factor of the respondents'treatment compliance scale scores(P<0.05).Preference for traditional treatment(external ointment,oral Western medicine,or traditional Chinese medicine)and disease in the progressive phase were the main negative influencing factors of the respondents'treatment compliance scale scores(P<0.05).The results of Pearson correlation analysis confirmed that the scores of the DCQ positively affected the scores of the MMAS-8(r=0.132,P=0.003).Conclusion Disease cognition and treatment adherence are suboptimal among psoriasis patients and are influ-enced by multiple factors.A clinical focus should prioritize rural patients with progressive disease,low income/education,and preference for traditional treatments.Strengthening public education on standardized medication and follow-up manage-ment is essential to enhance disease cognition and treatment adherence,facilitating comprehensive chronic disease manage-ment systems for psoriasis.
