Clinical value of inflammation burden index in predicting relapse in inflammatory bowel disease patients

Abstract:Objective To evaluate the clinical value of the Inflammation Burden Index(IBI)in predicting relapse in inflamma-tory bowel disease(IBD)patients,providing evidence for targeted preventive interventions.Methods A total of 182 IBD patients[78 Crohn's disease(CD),104 ulcerative colitis(UC)]admitted to Qingdao Hospital of University of Health and Rehabilitation Sciences(August 2019-September 2023)were enrolled.After 1-year follow-up,CD patients were assigned to relapse(n=35)and non-relapse(n=43)groups;UC patients to relapse(n=45)and non-relapse(n=59)groups.Clin-ical data,C-reactive protein(CRP),neutrophil(NEU),and lymphocyte(LYM)levels were collected,and IBI was calcu-lated.Receiver operating characteristic(ROC)curves were adopted to assess IBI's predictive value,and multivariate logistic regression was used to identify relapse risk factors.Results No significant differences existed in sex,age,body mass index(BMI),lesion site,smoking history,or history of alcohol use between relapse groups(P>0.05).The relapse group in CD patients exhibited significantly higher Crohn's Disease Activity Index(CDAI),erythrocyte sedimentation rate(ESR),NEU,CRP,and IBI but lower LYM and hemoglobin(Hb)than the non-relapse group in CD patients(P<0.05).The re-lapse group in UC patients showed significantly higher modified Mayo endoscopic scores,ESR,NEU,CRP,and IBI but lower LYM and Hb than the non-relapse group in UC patients(P<0.05).ROC analysis revealed IBI predicted CD relapse with area under curve(AUC)0.862(95%CI:0.817-0.912;cutoff 98.96;sensitivity 91.43%,specificity 69.77%)and UC relapse with AUC 0.784(95%CI:0.739-0.834;cutoff 71.08;sensitivity 88.88%,specificity 66.10%).Multivari-ate analysis identified CDAI score(OR=2.323,95%CI:1.477-3.654),CRP(OR=2.138,95%CI:1.292-3.539),ESR(OR=2.469,95%CI:1.413-4.317),and IBI(OR=2.948,95%CI:1.834-4.737)as influence factors for CD relapse(P<0.05).For UC relapse,modified Mayo score(OR=2.366,95%CI:1.427-3.922),CRP(OR=2.210,95%CI:1.425-3.428),ESR(OR=2.502,95%CI:1.454-4.306),and IBI(OR=3.136,95%CI:2.042-4.817)were influence factors(P<0.05).Conclusion IBI is an independent factor for relapse in IBD patients,which helps to iden-tify high-risk patients and provides a basis for guiding the clinical implementation of preventive interventions.
