Analysis of health literacy level and influencing factors among rural residents in Chongqing

Abstract:Objective To understand the health literacy level and its influencing factors among rural residents in Chongqing in 2022,providing decision-making support for relevant policy formulation.Methods Using multi-stage stratified cluster ran-dom sampling and probability proportional to size(PPS)sampling,permanent rural residents aged 15-69 in Chongqing were selected for questionnaire surveys.Descriptive analysis,χ2 test,and logistic regression analysis were employed to ex-plore health literacy levels and influencing factors among populations with different characteristics.Results The health lit-eracy level among rural residents in Chongqing in 2022 was 27.41%.Among the three dimensions of health literacy,health skills literacy(26.79%)was relatively low.Among the six categories of health literacy,chronic disease prevention and treatment literacy(23.88%)was relatively low.Univariate analysis results showed statistically significant differences in health literacy levels among rural residents with different ages,marital statuses,education levels,annual household income per capita,chronic disease status,and self-rated health status(P<0.05).Multivariate logistic regression analysis revealed that health literacy levels decreased with increasing age,with residents aged 65-69 having significantly lower health literacy than those aged 15-24(OR=0.210,95%CI:0.163-0.270).Married rural residents had higher health literacy levels compared to unmarried residents(OR=1.342,95%CI:1.133-1.588).Compared to illiterate/semi-illiterate residents,those with undergraduate education had the highest health literacy level(OR=34.464,95%CI:24.332-48.816).Resi-dents with annual household income per capita of over 3,000 yet under 5,000 yuan had the highest health literacy level com-pared to those with under 3,000 yuan(OR=1.318,95%CI:1.088-1.596).Regarding self-rated health,health literacy levels generally decreased as self-rated health declined;residents rating their health as "poor" had lower health literacy than those rating it as "good"(OR=0.549,95%CI:0.374-0.805).Conclusion Health literacy among rural residents in Chongqing has markedly improved in recent years but remains deficient in areas such as health skills and chronic disease pre-vention and treatment literacy.Enhancing health literacy requires comprehensive consideration of multiple factors and targe-ted health education strategies for different groups,particularly focusing on the elderly,those with low education,unmar-ried individuals,economically disadvantaged populations,and those with poor self-rated health.
