Characteristics and risk factors of re-rupture after admission in patients with intracranial aneurysms

Abstract:Objective To investigate the characteristics and risk factors of re-rupture after admission in patients with intracranial aneurysms,aiming to provide clinical treatment references.Methods A total of 102 patients with intracranial aneurysms ad-mitted to the Department of Neurosurgery at the First Affiliated Hospital of Air Force Medical University from February 2021 to February 2024 were selected.Patients were assigned to two groups based on whether re-rupture occurred after admission:the re-rupture group and the non-re-rupture group.Clinical data,including age,gender,smoking history,alcohol consumption history,history of cerebral infarction,hyperlipidemia,time from onset to admission,maximum aneurysm diameter,Hunt-Hess grade,and whether or not to have received invasive examinations,were collected using a self-made questionnaire.Results In the re-rupture group,the proportions of patients aged≥60 years,time from onset to admission ≥6 hours,systolic blood pressure upon admission ≥160 mmHg(1 mmHg=0.133 kPa),maximum aneurysm diameter ≥ 10 mm,Hunt-Hess gradeⅣ-Ⅴ,and those who underwent invasive examinations were higher than in the non-re-rupture group,with statistically signifi-cant differences(P<0.05).Binary logistic stepwise regression analysis revealed that time from onset to admission ≥6 hours(OR=2.167,95%CI:1.720-3.981),systolic blood pressure upon admission ≥ 160 mmHg(OR=3.804,95%CI:2.308-6.270),maximum aneurysm diameter ≥10 mm(OR=2.776,95%CI:1.758-4.383),Hunt-Hess grade Ⅳ-Ⅴ(OR=5.217,95%CI:2.453-11.096),and undergoing invasive examinations(OR=3.662,95%CI:1.722-7.788)were inde-pendent risk factors for re-rupture after admission(P<0.05).Conclusion The risk of re-rupture after admission in patients with intracranial aneurysms is influenced by multiple factors.Clinicians should focus on these high-risk factors and implement early interventions to reduce the incidence of re-rupture after admission.
