Factors influencing postoperative anxiety and development of a prediction model in patients receiving toric intraocular lens implantation

Abstract:Objective To analyze factors affecting anxiety in patients after toric intraocular lens(IOL)implantation for astigma-tism correction and to construct a predictive model to guide clinical decisionmaking.Methods We enrolled 120 patients who underwent toric IOL implantation between May 2022 and May 2024 at Tangshan Gongren Hospital.According to their postop-erative anxiety status,patients were assigned to an anxiety group(n=49)or a non-anxiety group(n=71).The general data and disease-related data of the two groups were compared.Multivariate logistic regression identified independent predictors of postoperative anxiety,which were then used to build a combined predictive model.Model performance was assessed via receiv-er operating characteristic(ROC)curve analysis.Results The anxiety group was significantly older,had a lower proportion a-ble to afford medical costs,and had lower percapita monthly household income than the non-anxiety group(all P<0.05).Pre-operative corneal astigmatism magnitude and visual analogue scale(VAS)pain scores after anesthesia wore off were significant-ly higher in the anxiety group(P<0.05).The anxiety group also had significantly higher rates of comorbidities(two or more),bilateral cataracts,and poor medication adherence(all P<0.05).Multivariate regression revealed that age,percapita monthly income,binocular cataracts,preoperative corneal astigmatism,and postanesthesia VAS score were independent pre-dictors of postoperative anxiety(P<0.05).The combined model yielded an area under the ROC curve of 0.916,outperfor-ming any single predictor(P<0.05).Ata cut-off of 0.557,the model's sensitivity and specificity were 96.37%and 65.51%,respectively.Conclusion A prediction model incorporating age,household income,binocular cataracts,corneal astigmatism,and post-anesthesia pain effectively identifies toric IOL patients at risk for postoperative anxiety.Early intervention targeting modifiable factors may improve anxiety management after surgery.
