Analysis of the current status and influencing factors of self-care agency in elderly patients with indwelling urinary catheters at home

Abstract:Objective To analyze the current status and influencing factors of self-care agency in elderly patients with indwell-ing urinary catheters at home,aiming to provide a reference for enhancing self-management levels in this population.Meth-ods A convenience sampling method was used to select 266 elderly patients who were discharged with indwelling urinary catheters from three tertiary hospitals in Shanghai between June and November 2022.General information questionnaires,Perceived Social Support Scale(PSSS),General Self-Efficacy Scale(GSES),Self-Rating Depression Scale(SDS),Self-Rat-ing Anxiety Scale(SAS),and Exercise of Self-Care Agency(ESCA)were administered.Pearson correlation analysis was conducted to examine the correlations of self-efficacy,perceived social support,and anxiety and depression levels with self-care agency in elderly patients with indwelling urinary catheters at home.Multiple linear regression analysis was performed to identify factors influencing self-care agency in this population.Results Univariate analysis revealed that female patients had higher ESCA scores than male patients(t=2.782,P=0.006);ESCA scores increased significantly with higher educa-tional levels(F=8.134,P<0.001);patients residing in urban areas had significantly higher ESCA scores than those in ru-ral areas(t=2.167,P=0.003);ESCA scores increased with higher monthly household income(F=4.319,P=0.005);ESCA scores increased with longer duration of indwelling catheters(F=9.127,P<0.001);and ESCA scores decreased with the number of combined chronic diseases(F=3.964,P=0.009).Pearson correlation analysis showed that self-effica-cy was positively correlated with self-care agency(r=0.701,P<0.05),while perceived social support,anxiety,and de-pression levels were negatively correlated with self-care agency(r=-0.686,-0.517,-0.499,respectively,all P<0.05).Multiple linear regression analysis indicated that self-efficacy,perceived social support,anxiety scale scores,dura-tion of indwelling catheters,number of combined chronic diseases,and monthly household income were significant influen-cing factors for self-care agency.Conclusion Elderly patients with indwelling urinary catheters at home generally exhibit in-sufficient self-care agency,influenced by physiological,psychological,and social factors.Support from hospitals,communi-ties,and government levels is needed to strengthen patients'self-care capabilities.
