Abstract:Objective To study the efficacy of single-incision Wiltse approach minimally invasive transforaminal lumbar inter-body fusion(MIS-TLIF)for degenerative lumbar canal stenosis in patients aged 80 and above. Methods Thirty-five pa-tients aged 80 and above with lumbar canal stenosis were treated with single-incision Wiltse approach MIS-TLIF.Intraopera-tive blood loss, length of postoperative hospital stay, time of operation, and complications were recorded.Changes in Visual Analogue Scale(VAS),Oswestry Disability Index(ODI),and Japanese Orthopaedic Association(JOA)scores before and after surgery were compared. The modified Macnab criterion was used to assess clinical effects one year post-operation.Re-sults Mean intraoperative blood loss was 346.85±28.57 mL,length of postoperative hospital stay was 5.22±0.94 days, and time of operation was 184.28±15.77 minutes.Wounds healed well without redness, swelling,or infection. Two cases of dural tear occurred.VAS and ODI scores decreased while JOA scores increased at three days, one month, and one year post-operation,with significant differences(P<0.05).The modified Macnab excellent and good rate was 94.2%.Conclu-sion Single-incision Wiltse approach MIS-TLIF is effective for degenerative lumbar canal stenosis in patients aged 80 and a-bove,offering good short-term efficacy with minimal trauma.
