Abstract:Objective To investigate the application value of combined detection of soluble triggering receptor expressed on myeloid cells-1(sTREM-1),fecal calprotectin(FC),serum amyloid A(SAA)proteins, and procalcitonin(PCT)in the differential diagnosis of pediatric infectious diarrhea. Methods A total of 120 children diagnosed with infectious diarrhea and treated at our hospital between June 2023 and December 2024 were selected and divided into a viral diarrhea group(n=60)and a bacterial diarrhea group(n=60)according to inclusion criteria.Another 60 healthy children who underwent routine physical examination during the same period were enrolled as the healthy control group.Clinical data and levels of sTREM-1,FC,SAA,and PCT were collected and compared among the three groups. Results Levels of sTREM-1,SAA,PCT,and FC were significantly higher in both the viral and bacterial diarrhea groups compared to the healthy group, and the levels were significantly higher in the bacterial group than in the viral group(P<0.05).Receiver operating characteristic(ROC)curves were generated using sTREM-1,SAA,PCT,and FC as test variables, with the healthy group as the reference. The area under the curve(AUC)for the combined detection in predicting bacterial and viral diarrhea was 0.977 and 0.868,re-spectively,both higher than those for individual markers(P<0.001).Conclusion Combined detection of sTREM-1,SAA,PCT,and FC provides high diagnostic value for pediatric infectious diarrhea, especially for bacterial infections. These findings offer a valuable reference for clinical diagnosis.
