Correlation between complements C4 and C5b-9 expression levels and mid-term MACE occurrence in patients with unstable angina pectoris after percutaneous coronary intervention

Abstract:Objective To investigate the correlation between complements C4 and C5b-9 expression levels and mid-term ma-jor adverse cardiovascular events(MACE)in patients with unstable angina pectoris(UAP)after percutaneous coronary in-tervention(PCI).Methods A total of 200 UAP patients undergoing PCI at our hospital from May 2021 to May 2022 were included.Preoperative serum levels of C5b-9 and C4 were measured using enzyme-linked immunosorbent assay(ELISA).The incidence of MACE was evaluated during the mid-term follow-up period(24 hours to 6 months post-PCI).Results Fol-low-up results showed that 14 patients experienced MACE,while 186 did not.The MACE group had significantly higher C4 levels and lower C5b-9 levels compared to the non-MACE group(P<0.05).Pearson linear correlation analysis revealed a positive correlation between C5b-9 levels and mid-term MACE occurrence(P<0.05).Receiver operating characteristic(ROC)curve analysis demonstrated that the sensitivity and specificity for predicting MACE were 57.50%and 57.10%for C4(AUC=0.575,cutoff=172.91 mg/L),and 86.00%and 85.70%for C5b-9(AUC=0.911,cutoff=431.117 μg/L).Based on cutoff values,patients were stratified into high-C4(≥172.91 mg/L,n=39)and low-C4(<172.91 mg/L,n=161)groups,and high-C5b-9(≥431.117 μg/L,n=172)and low-C5b-9(<431.117 μg/L,n=28)groups.The mean MACE occurrence time was shorter in the high-C4 group(5.03±0.03 months)than in the low-C4 group(5.55±0.09 months;Log-Rank χ2=63.397,P<0.05).Conversely,the high-C5b-9 group had a longer mean MACE occurrence time(5.69±1.06 months)than the low-C5b-9 group(5.04±1.07 months;Log-Rank χ2=6.143,P=0.013).Conclusion Complements C4 and C5b-9 expression levels are associated with mid-term MACE occurrence in UAP patients post-PCI and may serve as predictive biomarkers for MACE.
