A study of the clinical characteristics of depression in Parkinson's disease and its correlation with norepinephrine

Abstract:Objective To explore the clinical characteristics of depression Parkinson's disease(PD),risk factors and their cor-relation with norepinephrine.Methods 105 PD patients who attended the inpatient and outpatient departments of the Sec-ond Affiliated Hospital of Baotou Medical College from January 2021 to December 2022 were included in the study.The se-verity of depression was assessed based on the 17-item version of the Hamilton Depression Scale(HAMD),and they were categorized into 62 cases in the PD depressed group and 43 cases in the PD non-depressed group.All enrolled patients were recorded in detail of past history,clinical data,scale assessment,and peripheral blood norepinephrine level was detected.Results Compared with the PD non-depressed group,patients in the PD depressed group were more female,had more Pos-tural Instability/Gait Disorders(PIGD)type of PD,and had a higher H-Y staging,UPDRS-Ⅲ score,PIGD domain score,HAMD score,higher single-day levodopa equivalent dose,and lower norepinephrine level,and the difference was statisti-cally significant(P<0.05).Percentage of each factor of HAMD scale in PD depressed patients:compared with the stand-ard percentage of each factor,the percentage of blocking(0.24 vs.0.35)and anxiety/somatization factors(0.36 vs.0.39)was increased in PD depressed patients,and the percentage of the factors of cognitive impairment(0.24 vs.0.12)and weight loss(0.04 vs.0.02)was decreased(P<0.05).Multifactorial logistic regression analysis was performed,showing an increase in patients with PIGD-type PD(OR=3.363,95%CI:1.235-9.162,P=0.018),UPDRS-Ⅲ score(OR=1.076,95%CI:1.009-1.148,P=0.026),and norepinephrine(OR=0.999,95%CI:0.997-1.000,P=0.032)were risk factors for depression in PD.The norepinephrine level in PD patients was negatively correlated with HAMD score(r=-0.531)and UPDRS-Ⅲ score(r=-0.424)(both P<0.01).The ROC curve showed a high assessment value for the oc-currence of depression in PD patients when the UPDRS-Ⅲ score was≥23.5 and norepinephrine was≤1.47 nmol/L.Conclu-sion PD depression is dominated by blocking and anxiety/somatization symptom disorders,and patients with PIGD type PD,increased UPDRS-Ⅲ scores,and decreased norepinephrine are risk factors for PD depression,and dysregulation of nor-epinephrine transmitters is involved in the onset and development of PD depression.
