Application of the Three-Characteristics and Three-Standardized continuing care management in patients with diffuse large B-cell lymphoma

Abstract:Objective To explore the application effect of the Three-Characteristics and Three-Standardized continuing care management in patients with diffuse large B-cell lymphoma(DLBCL),aiming to provide a reference for the development of nursing management plans for these patients.Methods A total of 224 patients with DLBCL admitted to the Department of Hematology at Shangjin Hospital,West China Hospital,Sichuan University from January 2018 to March 2023 were selected and assigned to a control group and an observation group using a simple random method,with 112 cases in each group.The control group received conventional continuing care management,while the observation group received the Three-Character-istics and Three-Standardized continuing care management.The psychological resilience,self-efficacy,fatigue status,quali-ty of life,degree of satisfaction with nursing,and rehospitalization and recurrence within 1 year after discharge were com-pared between the two groups before and after intervention.Results Before intervention,no significant differences were noted in the scores of the three subscales(resilience,strength,and optimism)of the Connor-Davidson Resilience Scale(CD-RISC)between the two groups(P>0.05).The subscale scores of the CD-RISC scale in both groups increased significantly after intervention,and the observation group had significantly higher scores than the control group(P<0.05).Before inter-vention,no significant differences were found in the scores of the four subscales(disease,emotion,interpersonal,and coop-eration)of the Cancer Self-Efficacy Scale between the two groups(P>0.05).The subscale scores of the Cancer Self-Effica-cy Scale in both groups showed a significant increase after intervention,and the observation group had significantly higher scores than the control group(P<0.05).Before intervention,no significant differences were found in the scores of the four subscales(behavior,emotion,sensation,and cognition)of the Piper Fatigue Scale between the two groups(P>0.05).The subscale scores of the Piper Fatigue Scale in both groups showed a significant decline after intervention,and the observation group had significantly lower scores than the control group(P<0.05).Before intervention,no significant differences were observed in the four subscale(physiological,social,emotional,and functional)scores of the Functional Assessment of Cancer Therapy-General(FACT-G)scale between the two groups(P>0.05).The subscale scores of the FACT-G scale in both groups increased significantly after intervention,and the observation group had significantly higher scores than the con-trol group(P<0.05).The overall degree of satisfaction with nursing of patients in the observation group was significantly higher than that in the control group[98.21%(110/112)vs.87.50%(98/112);X2=9.692,P=0.002].The rehospital-ization rate and recurrence rate of patients in the observation group were 10.71%(12/112)and 3.57%(4/112),respective-ly,which were lower than the 26.79%(30/112)and 14.29%(16/112)in the control group,with statistically significant differences(x2=9.495,7.906;P=0.002,0.005;respectively).Conclusion The Three-Characteristics and Three-Stand-ardized continuing care management can effectively improve the psychological resilience and self-efficacy of patients with DL-BCL,reduce patient fatigue,raise quality of life,and achieve a high degree of satisfaction with nursing,which is worth pro-moting and applying.
