Predictive value of preoperative systemic immune-inflammation index for postoperative wound healing in patients with perianal abscess

Abstract:Objective To investigate the predictive value of preoperative systemic immune-inflammation index(SⅡ)for post-operative wound healing in patients with perianal abscess,providing a reference for clinical treatment.Methods A total of 152 patients who underwent perianal abscess surgery at Luzhou People's Hospital from January 2021 to August 2024 were selected as the study participants.Based on wound healing status two weeks postoperatively,the patients were allocated to a good healing group(n=118)and a poor healing group(n=34).Clinical data and preoperative SⅡ were collected.Changes in platelet count,neutrophil count,lymphocyte count,and SⅡ were analyzed,and their predictive value for postoperative wound healing was evaluated.Multivariate logistic regression was used to identify factors influencing postoperative wound healing.Results The poor healing group had significantly higher platelet counts,neutrophil counts,and SⅡ,but lower lymphocyte counts compared to the good healing group(P<0.05).The AUC(95%CI)for predicting postoperative wound healing using platelet count,neutrophil count,lymphocyte count,and SⅡ were 0.823(0.773-0.868),0.749(0.704-0.794),0.806(0.801-0.811),and 0.912(0.862-0.962),respectively.The poor healing group had signifi-cantly higher proportions of smoking history,diabetes,postoperative constipation,postoperative infection,and poor wound drainage compared to the good healing group(P<0.05).Multivariate logistic regression showed that diabetes(OR=2.040,95%CI:1.307-3.183),postoperative infection(OR=2.782,95%CI:1.265-6.116),poor wound drainage(OR=2.291,95%CI:1.258-4.174),and SⅡ ≥ 949.79(OR=4.807,95%CI:2.045-11.297)were influencing fac-tors for postoperative wound healing(P<0.05).Conclusion Elevated preoperative SⅡ is closely associated with poor post-operative wound healing in patients with perianal abscess and can serve as a potential biological marker for assessing postop-erative wound healing.
