Analysis of risk factors for systemic inflammatory response syndrome in patients with benign prostatic hyperplasia after transurethral plasmakinetic resection of the prostate

Abstract:Objective To explore the risk factors for systemic inflammatory response syndrome(SIRS)after transurethral plasmakinetic resection of the prostate(TUPKRP)in patients with benign prostatic hyperplasia(BPH),in order to provide a reference for clinical decision-making.Methods Clinical data of 170 BPH patients who underwent TUPKRP surgery at a hospital in Xinjiang from May 2019 to May 2022 were collected.Patients who developed postoperative SIRS were included in the SIRS group,and those who did not were included in the non-SIRS group.General information,laboratory test results,and perioperative indicators of the two groups of patients were collected.Univariate analysis and multivariate logistic regres-sion model were used to analyze the risk factors for SIRS after TUPKRP.Results Among the 170 BPH patients treated with TUPKRP,32 cases(18.82%)developed postoperative SIRS.There were statistically significant differences in age,presence or absence of combined type 2 diabetes mellitus(T2DM),preoperative prostate volume,presence or absence of preoperative urinary tract infection,preoperative serum albumin(Alb)level,preoperative hemoglobin(Hb)level,and op-eration time between the SIRS group and the non-SIRS group(P<0.05);whereas there were no statistically significant differences in body mass index,history of hypertension,preoperative International Prostatic Symptom Score(IPSS),preop-erative white blood cell(WBC)count,intraoperative blood loss,and postoperative catheter retention time between the two groups(P>0.05).The logistic regression model analysis indicated that advanced age(OR=1.194,95%CI:1.078-1.322),presence of combined T2DM(OR=3.142,95%CI:1.412-6.991),and large preoperative prostate volume(OR=1.087,95%CI:1.029-1.148)are independent risk factors for the occurrence of SIRS after TUPKRP(P<0.05),while high preoperative Hb level(OR=0.903,95%CI:0.838-0.973)is an independent protective factor for the occur-rence of SIRS after TUPKRP(P<0.05).Conclusion The incidence of SIRS after TUPKRP remains high.Advanced age,combined T2DM,and large preoperative prostate volume are independent risk factors for the occurrence of SIRS after TUP-KRP,while high preoperative Hb level is an independent protective factor.Targeted interventions can be made based on the above-mentioned factors to reduce the occurrence of postoperative SIRS.
