Relationship between serum Adropin,H-FABP,ANGPTL3 and myocardial remodeling in patients with coronary heart disease and chronic heart failure

Abstract:Objective To explore the relationship between serum Adropin,heart-type fatty acid-binding protein(H-FABP),angio-poietin-like protein 3(ANGPTL3),and myocardial remodeling in patients with coronary heart disease(CHD)and chronic heart failure(CHF).Methods A total of 115 patients with CHD and CHF who visited the First Affiliated Hospital of Xinjiang Medical University from January 2020 to June 2022 were retrospectively selected as the CHF group and 50 healthy volunteers who underwent physical examinations during the same period were selected as the control group.The levels of serum Adropin,H-FABP,AN-GPTL3,N-terminal pro-brain natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),and myocardial remode-ling-related indicators were compared between the two groups.According to the New York Heart Association(NYHA)functional classification criteria,patients in the CHF group were allocated to the Class Ⅱ group(n=49),Class Ⅲ group(n=42),and Class Ⅳ group(n=24).The levels of serum Adropin,H-FABP,ANGPTL3,NT-proBNP,and myocardial remodeling-related indica-tors in patients with different levels of heart function were compared,and the relationship between these three indicators and myo-cardial remodeling in patients with CHD and CHF was analyzed.Results The levels of serum Adropin,H-FABP,ANGPTL3,NT-proBNP,and left ventricular mass index(LVMI)in the CHF group were higher than those in the control group,while left ven-tricular end-diastolic diameter(LVEDD),left atrial diameter(LAP),and left ventricular posterior wall thickness(LVPW)were all greater than those in the control group.LVEF and left ventricular remodeling index(LVRI)levels were lower than those in the con-trol group,with statistical significance(P<0.05).The differences in levels of serum Adropin,H-FABP,ANGPTL3,NT-proB-NP,and LVEF in patients with different levels of heart function were statistically significant(P<0.05).With the increase of heart function level,the levels of serum Adropin,H-FABP,ANGPTL3,and NT-proBNP gradually increased,while the level of LVEF gradually decreased,and the differences between the groups were statistically significant(P<0.05).The differences in levels of LVEDD,LAP,LVPW,LVMI,and LVRI in patients with different levels of heart function were statistically significant(P<0.05).With the increase of heart function level,the levels of LVEDD,LAP,LVPW,and LVMI gradually increased,while the level of LVRI gradually decreased,and the differences between the groups were statistically significant(P<0.05).The levels of se-rum Adropin,H-FABP,and ANGPTL3 in the CHF group were positively correlated with LVEDD,LAP,LVPW,LVMI,NT-proBNP levels,and heart function classification(P<0.05);the levels of serum Adropin,H-FABP,ANGPTL3 in the CHF group were negatively correlated with LVRI and LVEF levels(P<0.05).Conclusion The levels of serum Adropin,H-FABP,and AN-GPTL3 increase with the increase of the heart function level in patients with CHD and CHF,which were closely related to myocardi-al remodeling.They can be used as important indicators for evaluating myocardial remodeling,and targeted interventions should be given to patients following the evaluation results.
