Bedside ultrasound measurement of gastric antral cross-sectional area and gastric juice volume in patients with post-hepatitis B cirrhosis

Abstract:Objective To investigate the correlation between antral cross-sectional area(CSA)measured by bedside ultra-sonography and gastric juice volume in patients with post-hepatitis B cirrhosis,and to provide evidence for clinical decision-making.Methods A total of 100 patients with non-post-hepatitis B cirrhosis and 100 patients with post-hepatitis B cirrhosis(sample 1)who underwent gastroscopy and 200 patients with post-hepatitis B cirrhosis(sample 2)who underwent gastros-copy were selected as the participants by convenience sampling method.Bedside ultrasound was used to measure and record the CSA of patients in sample 1;patients in sample 2 were randomly assigned to 5 groups.After the end of gastroscopy,a gastroscope was employed to suck the maximum degree of gastric fluid and gas by vacuum aspiration.An amount of 0 mL,50 mL,100 mL,150 mL,and 200 mL normal saline were rapidly injected into the stomach of patients through the gastro-scope biopsy orifice,respectively.The patients were asked to lie in the supine position within 2 minutes.Bedside ultrasound equipment was adopted to measure and calculate CSA.Results In the supine position,the mean CSA was 2.31~3.08 cm2(3.07±0.28)cm2 in patients with fasting non-post-hepatitis B cirrhosis and 2.60~7.26 cm2,(4.84±0.77)cm2 in patients with fasting post-hepatitis B cirrhosis.A significant difference was observed in the mean CSA between the two groups(P<0.05).Among patients of sample 2,there was a positive correlation between the amount of injected saline and CSA in su-pine patients after intragastric injection of different volumes of saline(rs=0.752,P<0.001).Conclusion CSA measured by bedside ultrasonography in patients with fasting post-hepatitis B cirrhosis in the supine position is significantly higher than that in patients with non-post-hepatitis B cirrhosis.Bedside ultrasound measurement of CSA in patients with post-hepa-titis B cirrhosis in the supine position can reflect the amount of gastric fluid and has certain clinical value in assessing the risk of perioperative aspiration in these patients,holding promise for a wide promotion in clinical anesthesia.
