Giant intracranial aneurysm with a large chronic perianeurysmal hematoma:a case report
Abstract:Case: A 61-year-old female patient presented with right ear tinnitus and facial twitching for 3 days, without impaired consciousness, limb convulsions, or incontinence. The symptoms lasted about 20 seconds and resolved spontaneously, with normal activity after relief. She had a history of brief epileptic seizures. Physical examination showed clear consciousness, appropriate answers, spontaneous eye opening, and the ability to follow commands. Neck stiffness and meningeal irritation signs were negative, and muscle strength and tone in all limbs were normal. She had a history of hypertension. Cerebrospinal fluid biochemistry showed slightly elevated protein (0.82 g/L). Electroencephalogram (EEG) revealed epileptiform discharges. Cranial MRI showed a mass-like lesion in the right temporal region with mixed signals on T1WI and T2WI, predominantly low signal on DWI with multiple susceptibility artifacts. The lesion had a relatively clear boundary, measuring approximately 8.1 cm × 4.5 cm × 5.9 cm, with a small amount of edema around it. After enhancement, the lesion margin showed obvious ring-like enhancement, and within it, a vascular tumor-like image with significant enhancement, measuring up to 1.5 cm in diameter, was connected to the right middle cerebral artery M1 segment, compressing surrounding structures.
