Meta analysis of quantitative dynamic contrast -enhanced MRI for the assessment of neoadjuvant chemotherapy in breast cancer

Abstract:Objective The purpose of this Meta-analysis was to determine the value of quantitative dynamic con trast-enhanced magnetic resonance imaging ( DCE-MRI ) in evaluating the response of breast cancer to the neoadjuvant chemotherapy ( NAC ) . Methods PubMed , PMC , OVID-MEDLINE , China National Knowledge Infrastructure ( CNKI ) , WANFANG, VIP datebases (from the building to November 1, 2016) were searched to collect articles about the therapeutic evaluation of NAC using the quantitative DCE-MRI in patients with breast cancer. The sensitivities and specificities of quantitative DCE-MRI in the evaluation of NAC for breast cancer were extracted from the articles. Meta-DiSc1.4 was ap-plied to evaluate the efficacy of the sensitivity and specificity in the Meta-analysis, a forest figure and summary receiver op-erating characteristics (SROC) were created. Results A total of 17 articles were enrolled in this study, including 854 cases in total, in which 255 cases were effective and the other 599 cases were ineffective to NAC, taking the pathological results as the gold standard. The sensitivity and specificity in the 14 articles of quantitative DCE-MRI (Ktrans, kep, ve) in comprehensive evaluating NAC for breast cancer were 83.3%(95%CI:78.3%-87.6%) and 82.9%(95%CI:79.5%-85.6%), respectively. The area under SROC was 89.99% (95%CI:85.70%-94.28%). The sensitivity and specificity in the 4 articles of Ktrans evaluating NAC for breast cancer were 83.6%(95%CI:71.2%-92.2%) and 81.6%(95%CI:71.0%-89.5%), respectively. The area under SROC was 89.99%(95%CI:83.33%-96.65%). Conclusion Our study confirmed that the quantitative DCE-MRI has great performance in monitoring NAC for breast cancer as its high sensitivity and specificity.
