The postoperative complications and cellular immune function features in patients with hepatic malignancy after radiofrequency ablation

Abstract:Objective To analyze the incidence of complications and the changes in cellular immune function of hepatic malignancies with different sizes after radiofrequency ablation.Methods Sixty-nine patients with hepatic malignancies who underwent radiofrequency ablation (RFA) in the Interventional Department of the Hunan Provincial People's Hospital from January 2013 to January 2015 were enrolled in this study.There were 52 males and 17 females,aged 37-76 years old and average age (48.92+11.29).A total of 23 healthy subjects who underwent routine physical exam in January 2015 were selected as the control group,with 17 males and 6 females,aged 37-76 years old and average age (48.02±11.14).The patients were divided into three groups according to the diameter of the tumors:Group 1 (diameter<3.0 cm),group 2 (3.0-5.0 cm in diameter),and group 3 (diameter>5.0 cm).Variance analysis was used to compare the curative effect and complications after treatment among the three patient groups.Student t test was used to compare the tumor markers (AFP)expression level between the preoperative patient group and the control group,and between before and after treatment in the disease groups.Variance analysis was used to compare the lymphocyte subgroup cell count among the control group,preoperative patient group,and postoperative patient group.Pearson correlation analysis was used to test the correlation between AFP and lymphocyte subgroup.Results The differences in complete ablation rate among the groups 1,2,and 3were statistically significant (P<0.05).There was no statistical difference in postoperative complications among the three patient groups (P>0.05).The level of AFP expression was significantly higher in the RFA preoperative and postoperative disease group than in the control group,and AFP was significantly lower in the postoperative disease group than in the preoperative period,P<0.05.The counts of CD3+,CD4+,CD8+,CD4+/CD8+,and NK cells of the RFA preoperative patient group were significantly lower than the control group and the postoperative disease group(P<0.05).The lymphocyte subgroup cell counts had no significant difference between the postoperative disease group and the control group (P>0.05).AFP was negatively correlated with the counts of NK cells,CD4+ cells,and CD3+ cells,and positively correlated with the count of CD8+cells,all P<0.05.Although AFP tended to be negatively correlated with CD19+ cells was negative (P>0.05).Conclusion RFA can effectively improve the cellular immune function of patients with hepatic malignancy,and the incidence of serious postoperative complications was low.RFA is worthy to be popularized in hepatic malignancy patients who can not tolerate surgical resection.
