Pseudoangiomatous stromal hyperplasia of the breast:clinical characteristics and imaging features

Abstract:Pseudoangiomatous stromal hyperplasia (PASH) of the breast is a benign mesenchymal proliferative lesion. It typically affects premenopausal women, Due to often complicated with other breast lesions, it is often overlooked and misdiagnosed. The exact etiology and pathogenesis of PASH is not clearly understood. There are not many imaging reports about PASH of breast. The main mammographic findings of PASH consist of a well-circumscribed, hyperdense mass without calcification. It usually present as well-circumscribed hypoechoic lesions on sonography. In MRI, the presence of slit-like spaces is the specific appearance of PASH, which shows low signal on T1WI, high signal on T2WI , and no enhancement in dynamic contrast-enhanced MRI (DCE-MRI). The DCE-MRI curve of PASH is typeⅠin most cases, and ADC values is in the range of benign breast lesions. On histologic examination, it is characterized by the presence of open slit-like spaces in dense collagenous stroma. Local surgical excision with adequate margins is the recommended treatment for PASH. It has an excellent prognosis, with no risk of malignant transformation and metastasis. The clinical, pathological, imaging and treatment of PASH were reviewed in this paper.
