Preoperative Hormone Therapy in Children with Hypospadias: A Clinical Review

Abstract:Hypospadias is a common congenital anomaly of male urethra in male pediatric patients.The mainstay treatment is hypospadias repair surgery.However,due to the common co-occurrence of microphallus,there could be not adequate tissue for proper reconstruction of the urethra.Many studies proposed that the preoperative androgen therapy definitively increased penile length and circumference which allows more tissue for operation.Since the randomized and controlled clinical trials on this topic are insufficient,the hormonal agent,the route of administration,dosing schedule and the timing of treatment are controversial.How to use the preoperative testosterone therapy,how to optimize the protocol,and how to avoid the adverse effects,are the topics of children urogenital surgery in recent years.This review summarized the clinical practice of preoperative testosterone therapy,including its benefits and risks in those children with hypospadias.
