Application value of 1∶1∶1 infusion of fresh plasma,platelets and red blood cells in patients with hemorrhagic shock

Abstract:Objective:To investigate the application value of 1∶1∶1 infusion of fresh plasma,platelets and red blood cells in patients with hemorrhagic shock.Methods:A retrospective analysis was performed on 129 patients with hemorrhagic shock admitted to Kaifeng People's Hospital from May 2022 to May 2024.According to different transfusion ratios,the patients were divided into control group(receiving 1∶1∶2 infusion of fresh plasma,platelets and red blood cells,n=64)and observation group(receiving 1∶1∶1 infusion of fresh plasma,platelets and red blood cells,n=65).Before blood transfusion and 1 day after blood transfusion,blood pressure,immune function,coagulation function[prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),thrombin time(TT),platelet(PLT)],mortality rate 1 day after blood transfusion and incidence of adverse reactions were compared between the two groups.Results:There were no significant difference indiastolic blood pressure,systolic blood pressure,IgA,IgG,IgM before and 1d after blood transfusion between the two groups(P>0.05);After blood transfusion,PT,APTT and TT were prolonged.FIB and PLT levels were decreased in the observation group and control group.Compared with the control group,PT,APTT and TT were shorter,but FIB and PLT levels were higher in the observation group(P<0.05);The incidence of adverse reactions and 1d mortality were 6.15%and 9.23%in the observation group,which were lower than the control group,14.06%and 15.62%,respectinely(P>0.05).Conclusions:Compared to the 1∶1∶2 ratio,early administration of fresh frozen plasma,platelets,and red blood cells at a 1∶1∶1 ratio significantly improved coagulation function in severe hemorrhagic shock patients with an acceptable safety profile.
