Analysis of clinical treatment of 45 patients with large area burn

Abstract:Objective: To summarize the experiences on treating the patients with large area burn.Methods: Forty-five patients with burn area of more than 50% TBSA admitted in our hospital were treated with fluid infusion, respiratory support, wound management, nutritional support, anti-infective therapy, and hormone and ulinastatin treatment. The age, burn area, length of hospital stay and complications of patients were compared according to their different prognosis.Results: Of the 45 cases, 26 were cured, 7 died and 12 gave up treatment. The age of gave up cases was obviously older than the cured ones (P<0.01), but no significant difference compared to the death ones (P>0.05). There was no obvious difference in the total body burn area among the three groups (P>0.05), but the III degree burn area in cured group was obviously smaller than in gave up group (P<0.01) and the died group (P<0.05). The main reasons of giving up treatment in 12 cases were serious illness, older age and heavy economic burden. The wound repair was basically completed within 6 weeks after treatment in the 26 cured cases with less than 5% area of residual wound. The main complications included sepsis, pericardial effusion, pleural effusion, pulmonary infection, infective endocarditis, irritable diabetes and reactive mental abnormalities. Bacterial culture of wound surface resulted for Pseudomonas aeruginosa (26 strains), Staphylococcus aureus (19 strains), Klebsiella pneumoniae (15 strains) and Acinetobacter calcoaceticus (15 strains). There were 3 cases died of burn sepsis, 4 cases died of giving up treatment by their family.Conclusions: The key point to the success treatment of patients with large area burn includes effective liquid infusion resuscitation, unobstructed airway, positive wound repair and powerful anti-infection treatment. Infection is the main cause leading to death in burns.
