A comparative study on characteristics of urinary tract infection in patients with or without diabetic mellitus

Abstract:Objective:To compare the clinical characteristics,pathogens and their drug sensitivity of urinary tract infections between patients with or without diabetes.Methods:One hundred and two diabetic patients with urinary tract infection (observation group)admitted from May 2012 to July 2014 were involved in the study,and 99 pa-tients with urinary tract infection but without diabetic mellitus who were admitted to our hospital during the same period served as controls (control group). The clinical data,pathogens,drug sensitivity and the prognosis were compared between the two groups.Results:Fifty-five patients (53.92%)presented as asymptomatic bacteriuria in the observation group,and it was higher than that of the control group (9 cases,9.09%;P<0.05). The number of patients with higher level of serum creatinine (43 cases,42.16%)or urine β2-microglobulin (49 cases, 48.04%)in observation group was much higher than those of the control group (15 cases,15.15% and 21 cases, 21.21%;P<0.05). In both groups,Escherichia coli was the main pathogen and it accounted for 57.46% in the observation group and 47.62% in control group. Escherichia coli in the control group showed higher sensitivity to sulperazon,cefepime hydrochloride for inj ection,imipenem and ceftazidine,whereas in the observation group, sulperazon,imipenem and ceftazidin were shown to be sensitive. Compared to the control group,the curative rate (31.37%)was lower and the hospital stay time [(20.51±10.17)days]was longer in diabetic patients than those of the observation group [90.91% and (15.16±11.43)days,P<0.05].Conclusions:Asymptomatic bacteriuria is found to be the main entity in diabetic patients with urinary tract infection,and the incidence of renal inj ury is ob-viously higher. For these patients,urine bacterial culture should be frequently to help select sensitive antibiotics for the treatment.
