Diarrhea in the ICU:epidemiological investigation and analysis of risk factors

Abstract:Objective:To investigate epidemiological characteristics and risk factors of diarrhea in the first 2 weeks of ICU stay.Methods:Two hundred and eighty-three patients admitted to ICU in Haidian Hospital during October 2013 to August 2014 were involved in the study. The basic data,the element of enteral nutrition (EN), and treatment drug were recorded. Multivariate and stratified analyses were performed to determine diarrhea risk factors.Results:A total of 1 603 patient-days were analyzed. Diarrhea was observed in 42 patients (15%)with a duration of 88 patient-days,and the incidence was 5.5/100 patient-days. The median day of onset of diarrhea was the 6th day,and diarrhea lasted for less than 4 days in 89% of patients (252 cases). Diarrhea risk factors were EN covering >60% of energy target (relative risk=1.75),use of antibiotics (relative risk=3.64)and antifungal drugs (relative risk=2.79). In patients receiving>60% of energy requirement by EN,diarrhea risk was increased by the com-bined use of antibiotics (relative risk=4.7)or antifungal drugs (relative risk=11.2)at the same time.Conclusions:Diar-rhea incidence during the first 2 weeks of stay in a tertiary comprehensive ICU is 1 5%.Diarrhea risk factors are EN cover-ing >60% of energy target,the use of antibiotics,and the use of antifungal drugs.
