Values of examination of multiple parameters in early diagnosis of diabetic nephropathy in patients with diabetes

Abstract:Objective:To investigate the diagnostic value of combination of blood glucose (Glu),urea,creatinine (Cr),uric acid (UA),β2-microglobulin (β2-MG),haemoglobin A1 c (HBA1 c),and microalbuminuria (MALB) in the diagnosis of early diabetic nephropathy(DN). Methods:Seventy-four patients with type 2 diabetic mellitus were divided into three groups according to the urinary albumin excretion rate (UAER):normal albuminuia group (n=28),early DN group(n=28)and proteinuria group (n=20). Fifty healthy individuals served as normal con-trol (normal control group). Glu,urea,Cr,UA,β2-MG,HBA1C,urine MALB levels and UAER were deter-mined in 74 patients with diabetes mellitus and 50 healthy controls. Sensitivity and specificity of single or combina-tion of parameters were analyzed.Results:The levels of Glu,Urea,Cr,UA,β2-MG,HbA1c,urine MALB levels and UAER in patients with diabetes mellitus were significantly higher than those of healthy controls (P<0.01). Compared with normal albuminuia group,the values of above parameters exceptβ2-MG were significantly higher in early DN group and proteinuria group (P<0.05). The sensitivity of a combination of the parameters for diag-nosis of DN was 97.6%,which was significantly higher than the sensitivity of each parameter. Conclusions:Com-bination of the determination of GLU,UREA,Cr,UA,β2-MG,HbA1c,urine MALB levels and UAER is help-ful for the diagnosis and prognosis of early DN.
