Effect of multimodal MRI and CT on overall efficacy and prognosis of patients with acute ischemic stroke beyond time window or with unknown onset time

Abstract:Objective To observe the effect of multimodal magnetic resonance imaging(MRI)and computed tomography(CT)on the overall efficacy and prognosis of patients with acute ischemic stroke(AIS)beyond the time window or with unknown onset time.Methods A comparison was made between the multimodal MRI cohort(MRI group)and the multimodal CT cohort(CT group)in terms of general clinical data,examination timeliness,and treatment efficacy.Comparative analyses were conducted for patients receiving endovascular treatment(EVT)in the MRI group versus the CT group,as well as for the prognosis of cases in the reperfusion therapy group versus those in the conventional medical therapy group.Results The MRI group required longer examination times compared to the CT group(P<0.001).However,no statistically significant differences were observed in treatment efficacy between the two groups or between patients receiving EVT within each group(P>0.05).In the MRI group,susceptibility-weighted imaging(SWI)identified 14 cases with susceptibility vessel signs(including 5 cases of two-layered susceptibility vessel signs),suggesting cardioembolic thrombi,and 23 cases with microbleeds.The CT group failed to demonstrate diagnostically significant findings in thrombus characterization or microhemorrhage detection.The prognosis[90-day modified Rankin Scale(mRS)score]of patients in the reperfusion therapy group was superior to that of the conventional medical therapy group(P<0.05).Conclusions Multimodal MRI is comparable to multimodal CT in evaluating the clinical outcomes of AIS beyond the time window or with unknown onset time,but it can provide more comprehensive imaging evidence.
