Application and efficacy analysis of bilateral puncture with level-by-level stratified injection of bone cement in percutaneous vertebroplasty

Abstract:Objective To explore the application and efficacy of bilateral puncture combined with level-by-level stratified injection of bone cement in percutaneous vertebroplasty(PVP).Methods From March 2022 to March 2024,a retrospective analysis was conducted on 102 patients with osteoporotic vertebral compression fracture(OVCF)with PVP treatment in the Department of Spinal Orthopedics,Zhuhai People's Hospital.The patients were categorized into two groups according to the method of cement injection:the experimental group(40 cases)received bilateral puncture with level-by-level stratified injection,while the control group(62 cases)used a unilateral approach with traditional injection.Statistical analyses were performed to determine various parameters,including the length of surgery,the amount of cement injected,the cement distribution score,postoperative adjacent vertebral fracture,cement leakage,vertebral body(anterior,central,and posterior margins)heights,Visual analogue scale(VAS)for pain,and Oswestry disability index(ODI).Results Compared with the control group,the experimental group exhibited a higher amount of bone cement injection(P<0.05),an enhanced cement distribution score(P<0.001),and a reduced cement leakage(P<0.05).Both groups exhibited significant improvements in postoperative vertebral body height(anterior,central,and posterior margin),VAS,and ODI compared with the preoperative period(P<0.001 or 0.05).Conclusion Both groups effectively alleviated patients'pain and improved their dysfunction in the treatment of OVCF.However,compared with the control group,the experimental group showed a higher amount of bone cement injection,better diffusion and distribution of the bone cement,reduced cement leakage,and potentially enhanced surgical safety.
