Retrospective study on the diagnosis,treatment and follow-up of 75 cases of the tumor at the sellar region
Abstract:Objective:To study the diagnosis and treatment of the tumor at the sellar region.Method:From 1987 to 2000,75 patients with tumor at the sellar region underwent an operation.Their morbidity rates,clinical and imaging manifestations,endocrine changes,operative and radio therapeutic efficacies and results of follow-up were retrospectively analyzed.Results:The pathological examination was made in all cases,and the results were given below:41(54.6%) of pituitary adenoma,20(26.7%)of craniopharyngioma,7(9.3%)of meningioma,2(2.7%)of teratoma,2(2.7%) of chordoma,1(1.3%) of germinoma,2(2.7%) of other malignant tumors.The clinical characteristics were as follows:(1) visual disturbance in 25 cases(37%);(2) endocrine changes in 27 cases(36%);(3) headache,vomiting and intracranial hypertension in 42 cases(56%),24 cases and 2 of 75 patients were treated in combination with postoperative radiotherapy and gamma knife,respedtively.The operative mortality was zero.The recurrence rate was 20% in the mean follow-up time of 3 years,and most patients have got satisfactory results.Conclusion:Initial symptoms,craniocerebral CT and MRI,and detection of pituitary hormones are the key to the preoperative diagnosis and operative design.The combination of aggressive surgical management and postoperative radiotherapy can improve the therapeutic efficacy.The minimally invasive microscopic technique is crucial for reducing post operative complications and achieving good results.
