Effect of early perioperative use of liposomal bupivacaine for nerve block on postoperative delirium in elderly pa-tients undergoing total hip arthroplasty

Abstract:Objective To evaluate the impact of early perioperative application of liposomal bupivacaine on post-operative delirium(POD)in elderly patients undergoing total hip arthroplasty(THA).Methods Elderly patients who underwent THA at Quanzhou First Hospital between June 2024 and March 2025 were included and randomly assigned to the liposomal bupivacaine group(observation group)or the lidocaine group(control group)using a random number table.The observation group received general anesthesia combined with liposomal bupivacaine nerve block,and the control group received general anesthesia combined with lidocaine nerve block.Postoperative delirium incidence within 5 days and post-operative pain scores were analyzed.Results The incidence of POD within 5 days postoperatively was significantly lower in the observation group compared with the control group[5 cases(7.7%)vs.15 cases(23.4%),P<0.05].Preoper-ative(admission)pain scores did not differ significantly between groups(P>0.05).At all postoperative time points,pain scores were significantly lower in the observation group(P<0.05).No significant differences were observed in base-line characteristics including sex,age,weight,height,BMI,ASA classification,and years of education(P>0.05).Conclusion Early perioperative use of liposomal bupivacaine in elderly THA patients reduces the incidence of postopera-tive delirium and effectively alleviates postoperative pain.
