Advances in the application of ultra-long-acting local anesthetics in thoracic and abdominal surgery

Abstract:With the widespread implementation of Enhanced Recovery After Surgery(ERAS)protocols,perioper-ative pain management has become a key component in thoracic and abdominal surgery.Traditional local anesthetics have a short duration of action(usually less than 8 hours),which is insufficient to cover the peak period of postoperative acute pain,leading to increased opioid consumption and higher risk of related adverse effects.Ultra-long-acting local anes-thetics achieve sustained drug release through lipid-based or polymer-based carrier systems,extending analgesic dura-tion to over 72 hours and significantly improving postoperative recovery quality.This review systematically summarizes the mechanisms of action,clinical applications,safety,and technical challenges of ultra-long-acting local anesthetics in thoracic and abdominal surgery,and discusses future directions to optimize perioperative analgesic strategies.
