Current status and prospects of ultra-long-acting analgesia in the perioperative period

Abstract:Postoperative pain management has long been a central focus in clinical practice.Current multimodal analgesia strategies mainly target the first 48 hours after surgery,often relying on opioids or conventional local anesthetics with relatively short durations of action.However,these approaches fail to address the need for sustained analgesia lasting 3-7 days postoperatively.Liposomal bupivacaine,the only ultra-long-acting analgesic currently available in clinical practice,achieves controlled release through multivesicular liposomal encapsulation.It has demonstrated certain efficacy in fascial plane blocks and peripheral nerve blocks.Although its superiority over conventional local anesthetics remains in-conclusive,its availability has broken the longstanding bottleneck of long-acting perioperative analgesia,marking a mile-stone in the field.In addition,other long-acting formulations such as bupivacaine gel solution(Posimir)and bupiva-caine implant(Xaracoll)have been approved,offering new options for perioperative pain control.Looking ahead,the fu-ture of ultra-long-acting perioperative analgesia will require a focus on achieving patient-acceptable pain states.Pro-gress will rely on innovations in drug delivery systems,the development of novel compounds,and integration with neuro-modulation techniques,thereby advancing analgesia strategies to improve patients' perioperative experience.
