Clinical characteristics and risk factors of lymphocyst and infection following radical hysterectomy for cervical cancer

Abstract:Objective To analyze the clinical characteristics of lymphocysts and associated infections following radical hysterectomy for cervical cancer,and to evaluate the risk factors for postoperative lymphocyst formation.Methods A retrospective analysis was conducted on 82 cervical cancer patients who underwent radical hysterectomy at Maoming Hospital of Traditional Chinese Medicine from January 2015 to June 2024.Patients were divided into a lymphocyst group and a non-lymphocyst group based on postoperative lymphocyst occurrence.Clinical symptoms and infections in the lym-phocyst group were documented.General information,such as age,comorbidities,and pathological features of cervical cancer,was compared between groups.Univariate and multivariate logistic regression analyses were used to assess risk factors for lymphocyst formation,and a predictive model was established.The predictive performance of the model was e-valuated using receiver operating characteristic(ROC)curve analysis.Results The incidence of lymphocyst formation after surgery was 25.61%(21/82).Most patients were asymptomatic(52.38%),followed by those with fever(28.57%)and abdominal pain(19.05%).Among the 21 patients with lymphocysts,6(28.57%)had infections.Sta-tistically significant differences were observed between the two groups in terms of drainage method,drainage duration,number of lymph nodes dissected,and vaginal stump closure technique(P<0.05).Multivariate logistic regression analy-sis identified the following independent risk factors:transvaginal drainage(OR=2.878,95%CI:1.577-5.251,P<0.05),drainage duration>5 d(OR=2.620,95%CI:1.375-4.990,P<0.05),>30 lymph nodes dissected(OR=3.476,95%CI:2.085-5.795,P<0.05),and vaginal stump closure(OR=4.406,95%CI:2.844-6.827,P<0.05).The predictive model was established as:P=1/(1+e-x),where x=-2.698+1.057 ×(drainage method)+0.963 ×(drainage duration)+1.246 ×(lymph nodes dissected)+1.483 ×(vaginal stump closure).The ROC curve analysis showed an area under the curve(AUC)of 0.883(P<0.05),indicating good predictive performance.Conclu-sion Most cases of postoperative lymphocysts in cervical cancer patients are asymptomatic,but a proportion may develop infections.Drainage method and duration,number of dissected lymph nodes,and vaginal stump closure technique are sig-nificant influencing factors for lymphocyst formation.The predictive model based on these factors is valuable for guiding clinical decision-making.
